Hospital staff have been 'disciplined' after a plaque bearing Prince Andrew's name was removed from a hospital chapel.

The plaque was removed from the chapel at Musgrove Park Hospital in Taunton, Somerset, in March this year.

It is understood that staff were asked to bring it back, but failed to do so.

An investigation has been launched into the incident, and a number of staff members have been disciplined.

A spokesperson for the hospital said: "We can confirm that an investigation has been launched into the removal of a plaque from the chapel at Musgrove Park Hospital.

"A number of staff members have been disciplined as a result of the investigation.

"We are unable to comment further at this time."

It is not known why the plaque was removed.

Prince Andrew has been under fire in recent months following his friendship with convicted sex offender Jeffrey Epstein.

The Duke of York has denied any wrongdoing, but he has stepped down from public duties.
